Primary Care

Mission Uplift enrolls all participants in the VA healthcare system, ensuring access to essential medical services. Primary Care includes physical exams, lab tests, medication management, preventive counseling, and referrals for both routine and urgent health needs.

Technology

We address the digital divide by supplying homeless veterans with necessary technology and support. This includes equipment and assistance for staying connected, managing virtual tasks, and accessing crucial electronic resources while following safety protocols.

Telehealth Services

We enhance access to telehealth by providing support for video conferencing, internet connectivity, and streaming media. This allows veterans to engage in remote consultations and receive timely healthcare from home. We will facilitate healthcare support and connectivity through Housing and Urban Development-Veterans Affairs Supportive Housing (HUD-VASH), Grant and Per Diem (GPD), Homeless Veterans' Reintegration Program (HVRP) and Supportive Services for Veteran Families (SSVF). 

 These programs help ensure that veterans have the necessary resources and support to access and benefit from telehealth services effectively. 

Care Practice Team

Our multidisciplinary team of physicians, RNs, social workers, case managers, and technicians offers holistic, person-centered care. We support the VA healthcare system with both business hour and on-call services, ensuring veterans receive comprehensive support as needed.
